My acne creams are effective on the spots I can see but I keep getting newer ones in other places. Do I need to start taking other acne treatments?

Not necessarily. The acne process affects large areas of your skin and not just the spots you see at any one time. You must remember that the early damage and blocking of the pores is under the skin and not visible.

Always use the acne products to the whole area that can be affected, and you will protect the spots at an early stage from ever becoming a problem as well as treating spots that you can see.

Does it matter when I use my creams?

It doesn't actually matter, so, if there is a regular time that is convenient for you, try that. Just consider how you will handle some of the side-effects - such as bleaching of your clothing or bedding by benzoyl peroxide. An odd side-effect with topical tetracyclines is that they fluoresce (glow) under ultraviolet (UV) light - so putting them on just before going clubbing is not a great idea.

The doctor has prescribed Dianette as the best acne treatment for my particular type of acne, but I am only 15 and my mother will go mental if she thinks I am on the pill. I don't even have a boyfriend, so why am I on a contraceptive?

Dianette is not just a contraceptive pill. It was actually designed to treat skin problems, so the contraception is really a side-effect! Acne is due to an over-sensitivity of your skin to normal levels of male-type hormones termed androgens. Even girls have a little bit of these.

Dianette contains a drug named cyproterone, which blocks the action of androgens, and this is why it can be useful with acne. It also contains estrogen, one of the female hormones, which adds to its effect and makes it a contraceptive. Only girls can use it, because boys would have lots of problems if their naturally much higher levels of testosterone were blocked and they were taking female hormones.

Share your problem with your mum so that she can understand why you are taking something that also acts as a contraceptive. If you are getting lots of spots just before your periods, you can tell her about the effect hormones have on your skin.

I have done really well with Dianette and want to keep taking it. My doctor says I must discontinue now that my skin is clear, as it has more of a risk than other contraceptive pills. Can I insist on staying on it?

This is a problem that many people with acne come up against. Although Dianette is also a good contraceptive, it is made to treat acne and the manufacturers recommend that it is stopped once it has worked. It may have a slightly higher risk of producing clots or raised blood pressure than some of the ordinary contraceptive pills but is usually safe to proceed with if you are otherwise fit and well and don't smoke. You have to realize, though, that this would be against the manufacturer's advice, and your GP is quite right to advise a change.

A recently introduced contraceptive pill called Yasmin contains an ingredient that seems to act in much the same way that the anti-androgen bit of Dianette does; it would make an excellent alternative if you are worried that your acne might recur if you stop taking the Dianette.
